The answer in two words is Urogenital Atrophy. Urogenital atrophy is a condition that affects most women as they go through menopause, and can result in lack of blood flow and secondary dryness in the vagina. If the vagina dries and is unable to produce moisture during intercourse, it can lead to painful intercourse, which down the road can lead to an avoidance of intercourse, then to a decrease of intimacy and finally to a relationship that is completely void of intimacy.
Vaginal dryness is a direct result of the body’s lack of estrogen as it approaches menopause. The vagina needs estrogen for its blood flow, for its thickness, for its suppleness, and for its ability to respond to intercourse by producing moisture. When women go through menopause this doesn’t happen. Not having moisture leads to painful intercourse. Many women have described it as knives coming through their vagina. Some women will continue painful intercourse to help with their relationship or because they don’t want to discuss it with their partner. Unfortunately, over time the frequency of intercourse starts to decrease, as does the intimacy, and then the relationship.
I find that women in 2018 don’t want this to happen. They have lovely relationships with their spouse or if single want to have relationships with partners. They remember having a good active sex life which they really enjoyed and they want it back. Certainly having atrophy will decrease this. As doctors, we need to be able to talk to our patients. I find that asking patients if they have any pain with intercourse or any trouble with intercourse, really helps them open up, so then we can have a conversation regarding what we can do about it. There are a number of solutions, both hormonal and non-hormonal which can really make a difference in your quality of life.
As women age, the vagina is becomes a victim of time and trauma. It suffers with childbirth, from hormonal changes, and from some diseases. There are a number of conditions resulting from an ageing vagina and one very annoying side-effect. Your bladder is a secondary victim.
After childbirth, the elasticity of the muscles on the pelvic floor weaken the vagina and the bladder causing numerous uncomfortable conditions.
